When a patient is not capable of making their own treatment decisions, healthcare providers turn to the patient’s substitute decision maker(s) to provide consent. Incorrect identification of the SDM has both legal and ethical implications.

This seminar will describe a quality improvement project aimed at improving the identification and documentation of SDM(s) for patients at Trillium Health Partners. The results from two pilot study units will be discussed. Next steps will be highlighted.
